Information and office assistance
The website provides general information about the practice. Its automated chat assistant answers office questions, describes the kinds of care Dr. Cody offers based on this website, and arranges office follow-up. Its replies are general information, may be imperfect, and are not individual advice. It does not provide medical advice, evaluate symptoms, diagnose conditions, recommend treatment, prescribe, or replace a qualified clinician. Do not use the website or chat for urgent care or emergencies. Call 911 for an emergency.
Appointments and responses
A chat, text, email or callback request is not a confirmed appointment and does not establish a doctor–patient relationship. The office must confirm appointments, services, fees, insurance participation and availability. Published hours may change. An available status indicates a staff workspace is active; it does not guarantee Dr. Cody can respond immediately. Responses and callbacks are subject to office availability.
Contact requests
Provide contact details only if you want the office to respond to your inquiry. Do not submit medical histories, sensitive documents, payment information, or another person’s information without permission. Agreeing to a callback authorizes a response about that request only, not marketing. Telephone, text and internet charges from your provider may apply.
Appropriate use
Use the website lawfully. Do not attempt to access staff accounts or other visitors’ information, submit malicious code, disrupt the service, or send abusive or unsolicited messages. Staff accounts are for authorized users only. Keep your sign-in details confidential.
Content and external services
Website content, photographs and branding belong to their respective owners. You may view and use the site for personal information and arranging care. External services, including maps, telephone, text and email services, operate under their own terms. Their availability is outside our control.
Availability and limitations
We work to keep information accurate and the site available, but errors and interruptions can occur. The website is provided as available. To the extent permitted by applicable law, we do not warrant uninterrupted or error-free operation and are not responsible for losses caused by reliance on general website content or third-party service interruptions. These terms do not limit rights or responsibilities that cannot legally be limited, including applicable patient rights or professional obligations.

Our Privacy Policy explains website information handling. These terms concern website use and do not replace clinical consent forms, the practice’s patient privacy notices, or agreements for care. We may update these terms by posting a revised effective date.
